Download Provider Package forms here • Application to Provide Supervised Private Home Day Care; • Medical Report for Day Care Provider; • Contract; • Police Record Check		• Attend & set up interviews with prospective providers • Monthly visits & safety checks every 3 months • Providers must pass a criminal reference check & medical exam • Alternate care when needed How do you apply for daycare placement for your child?
